Safeguard Dental Plan was designed by dental professionals to ensure that members receive preventative, restorative and specialty care when it is required. Each Safeguard Dentist must pass a rigorous screening process which includes verification of their credentials and on on-site inspection to make sure their facility, equipment and office practices meet Safeguard's high standards and regulatory requirements. It is this commitment to excellence that makes Safeguard Dental Plan the preferred choice for employers.

What are my premiums?
You have the option to pay the annual premium by check or credit card. You may also have it debited from your bank account or charged to your credit card on a monthly basis.*
* Your account will be debited/charged by the 15th of each month for one month's premium. A 10 day grace period is afforded to all members for any late fees or payment due before cancellation of benefits occurs.

How do I choose a dentist?
Please call Applied Financial's Customer Service toll free at (800) 419-1288 and let them know your are looking for a dental provider in your area as a prospective applicant for the Safeguard Dental Plan. Be sure to ask for the dental provider number and the provider's phone number. Then, place the provider number of the dentist you have chosen in the space provided on the enrollment form. If you ever want to change your dentist, simply contact us and we will assist you with choosing another Safeguard dentist in your area.
The Safeguard Dental Plan network is comprised of dentists who have been carefully screened and evaluated. Each provider is licensed through the State of Florida's Department of Business and Professional Regulations and monitored to verify O.S.H.A. standards, specifically for cleanliness and sterilization.
What if I need a specialist?
You have access to specialists in all fields of dentistry at reduced rates through the Universal Plan.
Safeguard members receive a 25% discount on all services provided by specialists.
Members may-self refer to a participating Safeguard Specialist. However, we recommend that you first visit your Safeguard General Dentist for an appropriate diagnosis. Safeguard representatives are available should you need assistance in selecting a Specialist.
What are other benefits?
The Safeguard Dental Plans include value-added benefits for Vision Care, Lasik Eye Surgery and Prescription Drugs at no additional cast to you.

How do I enroll?

When will my membership begin?
If your enrollment form and premium are received by the 20th of the month, you will be effective on the first day of the following month. Otherwise, your effective date is one month later. You will receive your membership card shortly before your effective date.
What if I have a dental emergency?
If you have a dental emergency, please contact your Safeguard General Dentist or our Customer Care Department immediately. Safeguard offers a $50 per claim/$100 per year benefit to assist members who require emergency care while temporarily traveling outside of their service area. Please see your Certificate of Coverage for details.
